We have developed a simple system to help you switch your accounts and automatic transactions to SMCU quickly and hassle-free. Once you've opened your SMCU Checking Account, the following forms will help you make the switch. Simply fill them out and mail or fax them to the appropriate company or your former financial institution:

  • Checklist - This form will help you ensure you've covered all your bases.
  • Close Account Form - Print, fill out and send this form to your previous financial institution to ensure they close your account and send you the balance from that account.
  • Change Automatic Withdrawal - Print, fill out and send this form to the company that is currently withdrawing funds from your account at your previous financial institution to ensure they stop withdrawals from that account and begin withdrawing funds from the SMCU account of your choice. This form requires your 12 digit account number which can be found at the bottom of your checks or on your statement. Click here for an example.
  • Change Payroll Direct Deposit - Print, fill out and send this form to your employer to ensure they stop depositing your paycheck from your account at your previous financial institution and begin depositing it into the SMCU account of your choice. This form requires your 12 digit account number which can be found at the bottom of your checks or on your statement. Click here for an example.
  • Deposit Slip - Download and print a checking account deposit slip for your ATM or mail-in deposit. You will need Adobe Acrobat Reader in order to download this file.

* Please note that children under 16 years of age are not eligible for checking accounts.
