Members may complete and mail applications to the credit union, bring them to any branch, or fax to the number on the form. Alternatively, you may apply online for any consumer loan, credit card or Home Equity Line of Credit.

Close Account Form - Print, fill out and send this form to your previous financial institution to ensure they close your account and send you the balance from that account.
Change Automatic Withdrawal - Print, fill out and send this form to the company that is currently withdrawing funds from your account at your previous financial institution to ensure they stop withdrawals from that account and begin withdrawing funds from the SMCU account of your choice. This form requires your 12 digit account number which can be found at the bottom of your checks or on your statement. Click here for an example.

Change Payroll Direct Deposit - Print, fill out and send this form to your employer to ensure they stop depositing your paycheck from your account at your previous financial institution and begin depositing it into the SMCU account of your choice.
